Is it their eyes, or is it dyslexia?

It is the question every parent asks when reading turns into a nightly battle. The honest answer involves a sentence our own industry does not always like repeating.

Vision problems do not cause dyslexia. The American Academy of Pediatrics, the American Academy of Ophthalmology and AAPOS say so jointly: "Vision problems can interfere with the process of learning; however, vision problems are not the cause of primary dyslexia or learning disabilities." But eye problems can absolutely make reading uncomfortable and hard to sustain, and some of them are invisible to a school screening. So do both: book a comprehensive eye exam, and ask the school for an educational evaluation. Run them at the same time rather than one after the other.

The short version

Two different problems that look identical at the kitchen table

A seven-year-old pushes the book away. She has read the same line three times. She says her eyes hurt, or she says nothing at all and just melts down. From the outside, an eye problem and a reading difference produce exactly the same scene — which is why so many families spend a year chasing the wrong one.

The distinction that actually helps is this: an eye problem makes reading physically uncomfortable or impossible to sustain. A reading difference makes decoding the words hard, however comfortable the child is. A child with an eye-teaming problem often reads well for five minutes and then falls apart. A child with dyslexia struggles on the first line, fresh, with no complaints about their eyes at all.

Parent pro tip: These are not mutually exclusive. A child can have dyslexia and convergence insufficiency, or dyslexia and uncorrected farsightedness. Finding an eye problem does not explain away a reading problem — and it is the second mistake families make after ignoring the eyes entirely.

What the evidence says

Dyslexia is a language problem, not an eye problem

In 2009 the American Academy of Pediatrics, the American Academy of Ophthalmology, AAPOS and the American Association of Certified Orthoptists published a joint statement in Pediatrics. It is unusually blunt for a consensus document. Dyslexia, it says, is a language-based disorder with genetic and neurological roots. Vision problems can interfere with learning, but they are not its cause.

The same statement goes further, and this is the part that matters commercially: "Scientific evidence does not support the efficacy of eye exercises, behavioral vision therapy, or special tinted filters or lenses for improving the long-term educational performance in these complex pediatric neurocognitive conditions." Those approaches, it concludes, "are not endorsed and should not be recommended."

On coloured overlays and tinted lenses specifically — sold under names like Irlen syndrome, visual stress or scotopic sensitivity — the evidence is not merely absent, it has been looked for and not found. Systematic reviews have concluded the apparent benefits are best explained by placebo, and the NHS in England does not routinely commission them for reading difficulty. The underlying diagnosis is not recognised in the World Health Organization's International Classification of Diseases.

What is real

Three eye problems worth ruling out first

Convergence insufficiency

The eyes struggle to turn inward together for close work. Symptoms, per the National Eye Institute, are blurry or double vision up close, tired or sore eyes, headaches, and trouble concentrating. The critical line: "You can have perfect vision and still have convergence insufficiency." A school screening tests distance letters. It cannot see this. Estimates of how common it is vary widely — commonly cited figures run from about 2% to 13% of school-age children, and a 2023 meta-analysis pooled to roughly 8%, though the underlying studies disagreed sharply.

Uncorrected farsightedness

The NEI-funded VIP-HIP study looked at 492 four- and five-year-olds and found that those with uncorrected moderate farsightedness scored lower on early literacy tests — a deficit of 4.3 points overall, and larger where near vision or depth perception was also affected. Note where the gap showed up: in print knowledge, the skill of recognising letters and words on a page. It did not show up in phonological awareness — the skill whose weakness defines dyslexia. The study's authors are explicit that whether correcting it fixes the gap has not yet been tested.

Plain uncorrected blur

The least interesting explanation and the most common one. A child who cannot comfortably resolve the print is a child who will avoid the print. This is the cheapest thing on the list to rule out and the fastest to fix, which is exactly why it belongs first. See 7 signs your child needs glasses.

The honest part

Treating an eye problem makes reading comfortable. It does not make a better reader.

This is the most useful thing in the research, and it comes from the people with the least incentive to say it.

The Convergence Insufficiency Treatment Trial, funded by the National Eye Institute, established in 2008 that office-based vergence therapy genuinely works for convergence insufficiency: among 221 children aged 9 to 17, 73% were successful or improved with office-based therapy, against 43% for home pencil push-ups and 35% for placebo. That is a real treatment for a real condition.

Then the same investigators asked the obvious follow-up: does fixing it improve reading? CITT-ART enrolled about 310 children aged 9 to 14 and, in 2019, published the answer. The therapy group improved their eye-teaming measures substantially. On reading comprehension, the therapy group gained 3.68 points and the placebo group gained 3.80 points. No difference.

The trial's lead investigator, Dr Mitchell Scheiman — an optometrist, publishing in an optometry journal, about a treatment his own profession provides — put it plainly: "While in-office vision therapy can improve visual function for children with CI, this trial indicates that clinicians should not suggest that it will lead to increased reading performance."

So: treating convergence insufficiency is worth doing. It makes reading comfortable and sustainable, which is not nothing when homework is a nightly fight. But if someone offers you vision therapy as a route to better reading scores, the trial that tested that claim found nothing.

A fair word on the disagreement: optometry and ophthalmology have argued about this for decades, and you will find both positions online. The optometric joint policy statement on vision and learning dates from 1996–98 and predates every trial cited here. Read closely, the two sides agree more than the argument suggests — both accept that vision problems can interfere with learning, and neither claims dyslexia is caused by the eyes. The disagreement is about how broad the category of "learning-related vision problems" should be.

What to do

The order that saves the most time

1. Book a comprehensive eye exam — not a school screening. It is fast, it is cheap relative to everything downstream, and it rules in or out refractive error, amblyopia, strabismus, convergence insufficiency and accommodative problems. A distance-acuity screening at school checks none of the last three. Any concern from you or a teacher is itself sufficient reason to go straight to a full exam. 2. Do not wait for the eye result before starting step three. The AAP is emphatic that early referral to educational professionals produces the best outcome. Running both in parallel costs nothing and saves months.

3. Request an educational evaluation from the school district, in writing. An eye doctor cannot diagnose dyslexia and should not try. That assessment — phonemic awareness, rapid naming, decoding, language, memory and attention — comes from a school or educational psychologist, a neuropsychologist, or a developmental-behavioural paediatrician.

4. Treat each thing with the treatment that matches it. Refractive error gets glasses. Convergence insufficiency gets vergence therapy. Dyslexia gets structured literacy — explicit, systematic instruction in phonemic awareness and phonics. None of these substitutes for another.

5. If they get glasses, make sure they actually get worn. The commonest failure of a prescription is a pair that is lost, broken, or quietly abandoned in a drawer because it hurts behind the ears. That part we can help with — building the routine without a daily battle and fixing the fit.

Can glasses fix dyslexia?

No. Glasses correct refractive error — blur caused by the shape of the eye. Dyslexia is a language-based learning difference, and the American Academy of Pediatrics, American Academy of Ophthalmology and AAPOS state jointly that vision problems are not the cause of primary dyslexia. Glasses can remove a physical obstacle to reading, which is worth doing, but they do not treat the reading difference itself.

Do coloured overlays or tinted lenses help children with dyslexia?

The evidence does not support them. The joint statement from the AAP, AAO and AAPOS says special tinted filters or lenses are not endorsed and should not be recommended. Systematic reviews of coloured overlays have concluded that apparent benefits are best explained by placebo, and the NHS in England does not routinely commission them for reading difficulty. Irlen or scotopic sensitivity syndrome is not recognised in the WHO International Classification of Diseases.

My child passed the school vision screening. Can I rule out an eye problem?

No. A school screening usually tests distance visual acuity only. The National Eye Institute states that you can have perfect vision and still have convergence insufficiency, which is a near-vision, eye-teaming problem. Screenings also miss most accommodative problems. If reading is a struggle, a comprehensive eye exam is the right next step regardless of a passed screening.

Does vision therapy improve reading?

It improves eye teaming, not reading. The CITT-ART trial randomised about 310 children with convergence insufficiency to vision therapy or placebo. Eye-teaming measures improved significantly with therapy, but reading comprehension gains were 3.68 points with therapy versus 3.80 with placebo — no difference. The trial's lead investigator, an optometrist, concluded that clinicians should not suggest vision therapy will lead to increased reading performance.

How common is dyslexia?

Estimates range from roughly 5% to 20% of children, depending on where the diagnostic line is drawn. The International Dyslexia Association says perhaps as many as 15 to 20% of the population show symptoms of dyslexia. Lower figures around 5 to 7% reflect stricter diagnostic thresholds. There is no natural cut-off point, which is why the numbers vary so much.

Who diagnoses what?

An optometrist or ophthalmologist diagnoses eye conditions — refractive error, amblyopia, strabismus, convergence insufficiency and accommodative dysfunction — and cannot diagnose dyslexia. Dyslexia is identified through an educational or psychological evaluation, usually by a school psychologist, educational psychologist, neuropsychologist or developmental-behavioural paediatrician. Your paediatrician can coordinate both referrals.
